Bloodline: Pureblood, Halfblood, Muggleborn, Squib, etc.
Sexuality: Heterosexual, Homosexual, Bisexual. (Keep it canon.)
Former House: Gryffindor, Hufflepuff, Slytherin or Ravenclaw.
Wand: Size, wood, core, flexibility.
Boggart: Greatest fear (try to keep it unrelated to family deaths).
Patronus: If they can cast it, what is it? If they can't cast it yet, what would it be?
Amortentia: The love potion's effect is different for everyone. What would your character smell?
Occupation: If original/dopple from the new world, what does your character do for a living?
Noteworthy: Special skills of any sort; great at non-verbal spells, Occlumency, Animagus, etc.

Appearance:
Give a good description of their appearance and mannerisms. Any cuts, bruises, scars to note?

Desired played-by: Make sure you have 2-3 options listed in accordance with desire to play. If your character is an original, feel free to list a single PB. Please provide four border-less icons of you choices.

Personality:
Give a good couple of paragraphs about what your character is like.

Positive personality traits:
List four, please.

Negative personality traits:
List four, please.

What motivates him/her:
Short or long, let us know what makes your character keep ticking.


Immediate Family:
If The Black Family, we know you have a huge family. Please simply state those your character cares about the most. Mothers, fathers, siblings and children.

History:
Write about what they've been doing since they left school. Try to give a few paragraphs summing up their history. A few paragraphs will do nicely, and in-depth is always appreciated.

Name/Alias: What can we call you by?
Are you over 18?: No need to tell us exactly how old you are, just prefer to know you are of age!
Preferred means of contact: Be it aim, icq, email or a screened contact post in your character journal. Let us know!
Time-zone: Let players know when to expect you to be around.
Quote: Give us a quote you like.

Plans/Ideas: Don't say you don't have any. You must have at least a minimum of three ideas as to what you'd like to do with your character. Some may be obvious especially if taking on a Premade. "I don't know " responses lead to nowhere.
Referral: Where did you find us? Who recruited you? Who can we display our thanks and gratitude toward?
Have you pimped us?: Have you tried to drag your friends in yet?
